All plans include a dental care benefit. We have three levels that companies can choose from, meaning that our plans can meet the diverse needs of your group. All dental care options include coverage for basic and major dental, and some plans include orthodontic coverage for children, and even adults.
Employers can select a plan that provides 80% or 100% coverage for eligible drugs. With no lifetime or annual maximums, your employees will be protected against rising drug costs.
All plans include a vision care benefit. We have three levels that companies can choose from, meaning that our plans can meet the diverse needs of your group. All vision care options include coverage for eye exams and prescription eyewear. Employees also have an additional $150 benefit for prescription safety glasses for on-the-job.

You are able to select a practitioner through the EFAP selection process based on your preferences and utilize the visits through this program. Under the Hour Bank Plan, companies can choose a flat Employee Life Insurance amount between $50,000 and $150,000. Office Supervisory Plan coverage is salary-based – participating companies can choose one, two or three times the employee’s annual salary. In the case of accidental death, the benefit doubles. Life insurance is $10,000 for a spouse and $5,000 for dependent children.
Benefits are available after 182 consecutive days of an employee’s total disability. The Hour Bank LTD benefit is a flat monthly benefit that the company selects. The Office Supervisory LTD benefit is based on the employee’s salary. Benefits are payable where an employee qualifies and was actively at work at the time of disability.
STD is an optional add-on to your company’s OpenCircle Benefit Plan. Benefits are payable on the first day of an employee’s accident or hospitalization, or on the eighth day of a disabling illness. The Hour Bank STD benefit is a flat weekly benefit that the company selects. The Office Supervisory STD benefit is based on the employee’s salary. Benefits are payable where an employee qualifies and was actively at work at the time of disability.
